New Student Registration
What to Expect
After the lottery in March 2025, families will be notified if their child has been offered a seat for the 2025-2026 school year or if they have been placed on the waitlist.
If your student has received an offer, you will be receiving email notification with the Registration Links and the Information you need to provide. Registration must be completed by May 31, 2025.
Step ONE:
Two Proofs of Residency will need to submitted in your online registration.
Proof 1: Please provide ONE photo from the below list
- Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098
- Mortgage papers
- Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts
Proof 2: Please provide ONE photo from the below list
- Most recent cable, gas, telephone, electric, and/or water bill
One-Time Fee (Due Every Year): $250.00
You may pay by Cash, Check, or Credit Card. We have a required one-time fee of $250.00 due every year, which covers registration, workbooks, curriculum, and one field trip. For families that qualify for Free it is $65.00, and for families that qualify for Reduced, it is $125.00.

- Or in person at Elgin Math and Science Academy, 1600 Dundee Ave. Elgin, IL 60120. Make checks payable to: EMSA, with your student’s first and last name on the memo line.
By completing the above registration steps, you will officially enroll your child for the 2025-2026 school year. If you need assistance, please call: (630) 883-5013.
Step TWO: FORMS NEEDED BY MAY 31, 2025.
NEW STUDENTS: Birth Certificate
NEW STUDENTS must submit an ORIGINAL, certified copy of your student’s birth certificate. This birth certificate may be obtained from the County in which your child was born. If you do not have a certified copy of your student’s birth certificate, please be sure to obtain one prior to May 31st. A staff member will need to see the certificate and make a copy on site to put in your student’s file.
ALL STUDENTS: Up-to-date Immunization Records (Due Every Year)

ALL STUDENTS: Recent Physical
All students entering EMSA in our first year are required to have a current physical examination completed on the approved Illinois Department of Public Health Form provided by your child’s physician. A current physical is one that was completed within one year of August 2025. A sports physical does not meet this requirement.
KINDERGARTEN AND NEW STUDENTS: Eye Exam
The eye exam is required for all students entering Kindergarten or students enrolling for the first time in an Illinois school.

KINDERGARTEN, 2ND GRADE AND 6TH GRADE STUDENTS INCLUDING NEW STUDENTS: Dental Exam
Optional FREE dental screenings will be available at EMSA. The dental exam is required for all students entering Kindergarten, 2nd and 6th grade students.

Free or Reduced Lunch Form
The Department of Education through the National School Lunch Program offers free and reduced price breakfast and lunches to students who qualify based on family income. Online forms are available here through our school lunch system, Titan. Paper forms are available at the front desk. You may also download the documents below. The free and reduced price meal program guidelines will also be used to determine eligibility for a student fees waiver*.
TO BE DETERMINED FOR THE 2024-2025 SCHOOL YEAR - THIS WILL BE SENT OUT JULY WITH THE UPDATED COST.
Uniforms may be purchased online through our uniform shop through French Toast.
