New Student Registration
What to Expect
After the lottery in March 2026, families will be notified if their child has been offered a seat for the 2026-2027 school year or if they have been placed on the waitlist. If your student has received an offer, you will receive an email notification with the Registration Links and the Information you need to provide. Registration must be completed by April 15, 2026.
All documents required are listed below.
Step ONE:
Two Proofs of Residency will need to be submitted in your online registration.
Proof 1: Please provide ONE photo from the list below.
- Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098
- Mortgage papers
- Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts
Proof 2: Please provide ONE photo from the list below.
- Most recent cable, gas, telephone, electric, and/or water bill
One-Time Fee (Due Every Year): $250.00
You may pay by Cash, Check, or Credit Card. We have a required one-time fee of $250.00 due every year, which covers registration, workbooks, curriculum, and one field trip. For families that qualify for Free Lunch, it is $65.00, and for families that qualify for Reduced Lunch, it is $125.00.

- Or in person at Elgin Math and Science Academy, 1600 Dundee Ave. Elgin, IL 60120. Make checks payable to: EMSA, with your student’s first and last name on the memo line.
Step TWO: FORMS NEEDED BY APRIL 15, 2026.
NEW STUDENTS ONLY: Birth Certificate
NEW STUDENTS must submit an ORIGINAL, certified copy of your student’s birth certificate. This birth certificate may be obtained from the County in which your child was born. If you do not have a certified copy of your student’s birth certificate, please be sure to obtain one before April 15, 2026. A staff member will need to see the certificate and make a copy on-site to put in your student’s file.
ALL NEW STUDENTS, 2ND AND 6TH GRADE:

ALL NEW STUDENTS: Recent Physical Required
All students are required to have a current physical examination completed on the approved Illinois Department of Public Health Form provided by your child’s physician. A current physical is one that was completed within one year of August 2026. A sports physical does not meet this requirement.
ALL STUDENTS: Eye Exam Required
The eye exam is required for all students.

School Medication/Treatment Authorization
ALL STUDENTS: If your student has medical plans such as (Allergy, Asthma, etc.), you must provide the proper medical documentation from the doctor's office along with the action plan, and medical authorization form (if applicable).

Free or Reduced Lunch Form
The Department of Education, through the National School Lunch Program, offers free and reduced-price breakfast and lunch to students who qualify based on family income. Online forms are available through our school lunch system, MealTime.
TO BE DETERMINED FOR THE 2026-2027 SCHOOL YEAR - THIS WILL BE SENT OUT IN JULY WITH THE UPDATED COST.
